If you are interested in working with a provider at The EBP for an evaluation, treatment, or consultation please contact us by phone or email.

Our Receptionist or Clinical Executive Director will be able to answer your questions and provide you with important information regarding our practice, services, and fees.  You will be asked some brief questions to determine which of our providers will be able to address your specific needs.

If you decide to schedule an appointment, The EBP will send you some documents to complete prior to your first appointment.
